PREVENTIVE PHYSICAL THERAPY FOR KNEE REPLACEMENT RECOVERY AND LONG-TERM MOBILITY

What to Expect from Physical Therapy After a Knee Replacement

Recovering from a knee replacement can feel overwhelming, but understanding the rehabilitation process can help you regain mobility and confidence. At Emery Physical Therapy, our team in Mount Prospect, Des Plaines, Oakbrook Terrace, and surrounding areas specializes in guiding patients through every stage of recovery, from early mobility to returning to daily activities safely.

Understanding the Early Recovery Phase

Following knee replacement surgery, it’s normal to experience swelling, bruising, and some discomfort. These are expected parts of the healing process. Your surgeon may provide pain medication to manage soreness, which can help you participate more fully in your therapy sessions. Early physical therapy focuses on gentle movement to reduce stiffness, improve circulation, and promote healing.

Most patients are able to go home the same day or the day after surgery. Before discharge, your medical team ensures you can safely stand and walk with an assistive device, such as a walker or crutches. Physical therapy may begin in your home during the first one to two weeks, allowing you to work with a therapist in a familiar environment while your knee is still sensitive to activity.

Home-Based Therapy: Building the Foundation

During home therapy, the primary goal is restoring range of motion and preventing stiffness. Therapists at Emery Physical Therapy will guide your knee through bending and straightening exercises, monitor swelling, and provide strategies for safe movement at home. This phase also includes light strengthening exercises for surrounding muscles to support the joint. Patients are encouraged to continue exercises independently between sessions to maximize recovery.

Transitioning to Outpatient Physical Therapy

Once you transition to our outpatient clinic, therapy becomes more structured and focused on regaining strength, flexibility, and functional mobility. An initial evaluation helps our therapists measure your knee’s range of motion, assess muscle strength, and develop a personalized plan of care.

Treatment often begins with low-impact cardiovascular exercises, such as stationary biking or using a Nu-Step machine, to increase blood flow and gently move the knee. Manual therapy techniques, including gentle massage and kneecap mobilizations, may be used to prevent scar tissue formation and improve flexibility. Strengthening exercises target the quadriceps, hamstrings, and hip muscles, preparing you to safely perform daily activities like walking, climbing stairs, and getting in and out of cars.

Milestones and Goals in Rehabilitation

During physical therapy, therapists focus on key milestones that indicate progress:

  • Achieving full knee extension to restore normal walking mechanics

  • Reaching 90 degrees of knee flexion within the first two weeks

  • Progressing to 120 degrees of knee flexion for daily functional activities

Rehabilitation typically lasts several weeks to a few months, depending on individual recovery speed, activity level, and personal goals. While full recovery may take six months to a year, most patients regain the ability to perform essential daily activities much sooner. Some occasional soreness or joint noises are normal and not usually a sign of complications.

Pre-Surgery Physical Therapy: Pre-Hab

Engaging in physical therapy before surgery, known as “pre-hab,” can significantly improve post-surgical outcomes. Strengthening the knee and improving flexibility before a replacement can reduce pain, enhance mobility, and accelerate recovery. Pre-hab also helps patients maintain independence and delay surgery if symptoms are manageable.
